- Fill it in online OR
- Download the form here in DOC or PDF, fill it in and fax to 086 670 1849 OR scan and email to the office.
- Pay your membership fees (Graduate membership R100pm or Student membership R25pm / R50pm)
- Pay via Givengain OR
- Deposit directly in the CMF account (reference payment with your SURNAME and send proof of payment to the office)
